The patient was hit in the groin whilst playing cricket. He complained of a swollen and painful right scrotum
The testicular capsule is disrupted. There is an associated hydrocoele and scrotal skin thickening.
The patient underwent surgical exploration which confirmed the rupture with extrusion of seminal tubules.
Ultrasound may sometimes clearly show disruption of the capsule and extrusion of the seminal tubules. In other cases, only an irregularity of the capsule and a haemorrhagic hydrocoele may be seen, but in these cases rupture should be presumed.
